North Shore-LIJ Investing $175M in Cancer Institute Expansion

The North Shore-LIJ Health System announced today it is making an initial investment of $ 175 million to expand its cancer services throughout the New York metropolitan area. In establishing the North Shore-LIJ Cancer Institute, the health system is opening and expanding cancer treatment centers throughout Long Island, Queens, Manhattan and Staten Island.

Historically, North Shore-LIJ's cancer services have been delivered by its individual hospitals, working through their full-time hematologists and oncologists, and affiliated community based physicians, who collectively treat more than 16,000 new cancer patients every year -- more than any other hospital in the metropolitan area. As part of the health systems new initiative, the North Shore-LIJ Cancer Institute now has signed on approximately 150 physician members throughout the New York area in 20 cancer specialties.

"Cancer is the second-leading cause of death in this country and aging baby boomers will increase the demand for cancer services, said Daniel Budman, MD, co-director of the North Shore-LIJ Cancer Institute, and chief of hematology/oncology at North Shore University Hospital and LIJ Medical Center. Bringing together the tremendous talent and expertise into one group will reduce fragmentation and improve the coordination of multidisciplinary care across the continuum."

To establish a broader regional presence, the North Shore-LIJ Cancer Institute is investing $ 175 million over the next several years to open new cancer treatment centers and expand existing ones throughout the New York area.

A $ 67 million construction project is already underway at North Shore-LIJs Center for Advanced Medicine (CFAM) in Lake Success, where 61,150 square feet of interior space is being redeveloped contiguous to the current Monter Cancer Center. Nearly 40,000 square feet are being added to the current 37,000-square-foot Monter Center, which has been serving North Shore University Hospital's (NSUH) oncology outpatients since 2006. It will now be the new home for the fully integrated ambulatory hematology/oncology and chemotherapy treatment services delivered by both NSUH and LIJ Medical Center.

In addition, radiation medicine and surgical, gynecologic and neuro oncology services now delivered at LIJ Medical Center and NSUH will be relocated to CFAM, where nearly 50,000 square feet of space has been set aside for three linear accelerators, a gamma knife, stereotactic radiation and brachytherapy services. Diagnostic radiology, breast imaging, uro-oncology and ambulatory surgery programs are already based at CFAM, providing a fully integrated spectrum of comprehensive cancer prevention, diagnosis, treatment, genetic counseling, nutritional and social work, supportive care and survivorship under one roof all under the auspices of the Cancer Institute.

Skin Cancer Detection and Prevention Month: Skin Check-ups and Laser Technology Team Up in the Fight Against Skin Cancer

One in five Americans will develop a type of skin cancer at some point in life; annual indoor tanning increases that chance by 20 percent. There are several different types of skin cancer, each with its own unique characteristics. Skin cancers include basal cell carcinoma, squamous cell carcinoma, and the most serious form, melanoma. Melanoma is both serious and difficult to treat, especially in later stages. According to the American Academy of Dermatology, it is important to check for skin cancer at least once a year with full body check by a knowledgeable dermatologist. It is also recommended that suncreen with SPF 30 or higher be worn as a first line of defense.

Precancerous cells, such as actinic keratosis are the first indicators of danger. It is a scaly lesion most commonly found on the face and back of the hands of those with fair skin, which can be treated using a laser technology called Fraxel Re:store Dual. The Fraxel Re:store Dual laser is a non-invasive resurfacing system used to treat wrinkles, loose skin, sun damage, and is FDA-approved to treat precancerous lesions before they become life threatening. Basal cell carcinoma (BCC) is the most common type of skin cancer in the United States, it also grows slowly and rarely metastasizes (spreads to other areas of the body). However, if left untreated, basal cell will continue to grow, invade and destroy the surrounding skin and tissues beneath the skin. The major cause of developing basal cell carcinoma is overexposure to ultraviolet light; from either the outdoors natural light of the sun or from indoor tanning beds.

While it is important for people to be able to recognize a basal cell carcinoma growth at home, it should still be examined and confirmed by a dermatologist because it is often mistaken for other skin conditions such as acne, cysts, eczema or psoriasis. It appears as a pink or pearly white lesion or sore that can easily bleed, crust over or heal poorly. They are characterized as dome or nodule shapes, flat growths, or scar-like lesions.

Squamous cell carcinoma (SCC) is the second most common form of skin cancer in the United States. The most common cause of squamous cell carcinoma is also the overexposure to ultraviolet light from the sun or tanning beds. It can develop anywhere along the body, although the most prevalently exposed areas, such as the head, neck, hands, shoulders, and lower legs, are the most common. Squamous cell carcinoma lesions appear as hard, superficial, dark red growths that become dome-shaped as they get larger and can sometimes look like dry skin patches.

The most infamously dangerous type of skin cancer is melanoma, which accounts for more than 9 to 12-thousand skin cancer deaths each year and can appear on any area of the body. In the United States, it is the 5th most common cancer in men, and the 7th most common cancer in women. Melanoma typically resembles a mole; however if the pigmentation of the area is not uniform or if the diameter is greater than 6 millimeters, it should be examined by a dermatologist. Surgery for melanoma can be effective only if the melanoma is caught early.

Treating basal and squamous cell carcinoma depend on many factors including the type, location and age of the growth. One method is cryotherapy, which involves killing the cancerous cells by freezing them with liquid nitrogen. However, it is most effectively treated by the MOHS Micrographic surgery procedure. The lesion is removed and examined under a microscope to ensure complete removal of the cancerous cells. Because the skin is removed in careful layers, the technique salvages as much healthy skin as possible. The MOHS Micrographic surgery is especially useful in areas of cosmetic concern, such as the face. Regular checkups should follow afterwards to guarantee minimalistic scarring and healthy healing process. Physicians who can perform MOHS surgery, such as Dr. Rokhsar, must be specially licensed with one additional year of training.

The Manhasset Womens Coalition Against Breast Cancer (MWCABC) on Friday presented a $ 90,000 check to Feinstein Institute for Medical Research investigators Iuliana Shapira, MD and Annette Lee, PhD. The money will go directly to the Breast and Ovarian Specimen Bank, a joint research endeavor of the Feinstein Institute and North Shore-LIJ Cancer Institute. Drs. Shapira and Lee established the specimen bank in 2010 to serve as a vital resource for discovering and validating new biomarkers for the early detection of breast and/or ovarian cancer, as well as predicting the disease course and outcomes, treatment response and risk of relapse.

The generosity of this gift is surpassed only by the love and caring of the thoughtful and foresighted women of the Manhasset Coalition who bestowed it upon us, said Dr. Shapira, MD, a Feinstein investigator who also serves as director of the Cancer Genetics Program at the North Shore-LIJ Cancer Institute.

The Manhasset Womens Coalition has been a tremendous force in our community, said Kevin Dwyer, senior vice president and chief development officer at North Shore-LIJ. The two investigators initially teamed up to conduct a first-of-its-kind study dedicated to improving the survival rates of women diagnosed with breast and ovarian cancer. The MWCABC funding will go to support their investigation of Plasma microRNAs as biomarkers for predicting the benefit of surgery and adjuvant or neoadjuvant chemotherapy to treat breast cancer. Preliminary studies from the laboratory suggest that micro RNA, a genetic marker embedded in deoxyribonucleic acid (DNA), indicates whether an ovarian tumor is benign or malignant, and whether the patient will benefit from chemotherapy after the surgical removal of the tumor. Earlier this year, they were awarded the Alison Conyngham Neisloss Collaborative Women in Science Award, given by the Feinstein Institutes Advancing Women in Science and Medicine (AWSM) organization.

Since their initial project received Institutional Review Board approval in November of 2010, more than 350 women have been enrolled in the breast ovarian cancer tissue bank. Recent published findings identified four main subtypes of breast cancer, one of which has distinct similarities to ovarian cancer. This makes their research efforts to simultaneously study both breast and ovarian cancer even more important in breast cancer research.

The Manhasset Womens Coalition Against Breast Cancer (MWCABC) is an all-volunteer, non-profit charitable organization founded in 1997 to unite the women of Manhasset, Long Island in the fight against breast cancer. MWCABC has been supporting North Shore-LIJ since 2004, providing over $ 300,000 in philanthropic support for North Shore-LIJs breast cancer research and supportive care programs, in addition to palliative care. The $ 90,000 gift is the Coalitions largest gift to date. This gift includes $ 10,000 contributed by the Junior Coalition of MWCABC, high-school aged members who run their own fund raising efforts for financial support of North Shore-LIJ programs, as well as share holiday treats each year with families on the North Shore University Hospital Palliative Care Unit. The MWCABC is also a significant referral source within the Long Island community, with strong efforts in this area by board member Dorothy Forte, who plays a key role in the Coalitions ongoing activities.

Five Easy Tips to Help Prevent Skin Cancer: Saving Skin from the Endless Summer Sun

Florida is the state of the endless summer sun. But that also puts residents at a higher risk of sunburns and much more. This May is Skin Cancer Awareness Month and with an estimated 3.5 million cases of skin cancer diagnosed each year, Board Certified Radiation Oncologists at the Pinellas cancer treatment center, WellSpring Oncology, are spreading the message of prevention and awareness.

Skin cancer is affecting younger and younger people, explains Robert Miller, M.D., Board Certified Radiation Oncologist at WellSpring Oncology. Melanoma is the most common form of cancer for people ages 25-29. This is a disease you have to prevent from day one. Just one blistering sunburn in youth more than doubles your risk of melanoma.

But Dr. Miller isn't suggesting skipping the beach entirely, just to be more prepared when heading out with these five tips.

1. Check the Clock: The summer sun gives off the most UV rays from 10 a.m. to 4 p.m. Dr. Miller suggests finding shade or staying indoors during these hours.

You don't have to be laying out to get a sunburn, even walking to and from your car can expose your skin to UV rays, Dr. Miller explains.

2. Block the Burn: No matter what time of day it is remember to block the burn with sunscreen.

We tell our patients at WellSpring Oncology to use SPF 30 for any outdoor activity. This will keep out about 97 percent of the sun's UVA and UVB rays and give you a safe way of enjoying the warm weather, Dr. Miller says.

Sunscreen should be applied one-half hour before going outside, giving the skin time to absorb it. Because sunscreen tends to be broken down over time by the sun, and rubbed or washed off with sweating and water exposure, it should be reapplied at least every two hours outdoors, and immediately after swimming or heavy sweating. At least one ounce (two tablespoons) is needed to cover the entire body surface.

3. Top off Your Look: Hats aren't just for fashion, according to Dr. Miller. Wide-brim hats provide protection for the ears, nose, shoulders and scalp from sunburns.

Shielding your eyes from the sun is also crucial to preventing any UV damage. So don't forget the sunglasses! Dr. Miller warns.

4. No Tanning Beds: Many used to think tanning beds were the safe way to get that summer glow, says Dr. Miller. But now we know there is no safe tan.

The Skin Cancer Foundation warns indoor tanners are 74 percent more likely to develop melanoma than those who have never tanned indoors. As a safe alternative, Dr. Miller suggests opting for spray-on tan instead.

5. Check the Mirror: It's important to know your own risk factors, explains Dr. Miller.

That includes fair skin, family history and prior blistering sunburns. WellSpring Oncology encourages self-exams by canvasing the skin and looking for atypical moles or freckles and documenting any changes.

The faster we catch the cancer, the more likely we'll be able to eradicate it completely, Dr. Miller says.

Darlene Moore of West Palm Beach, Fla., echoes Dr. Platzers sentiment of the importance of diagnosing melanoma early. During a regular skin cancer screening at Waters Edge Dermatology, Shanna Stone, ARNP, saw a few areas of concern on Ms. Moores ear and arm and took biopsies. Ms. Moore was diagnosed with Stage 1 melanoma on her arm and basal cell carcinoma on her ear.

The basal cell, frankly, I wasnt as concerned about, says Ms. Moore. Dr. Platzer reassured me that Mohs surgery was very effective for basal cell carcinoma and he was able to get it all. But I was really concerned about melanoma, because my brother-in-law died of melanoma.

Surgery is the best way to remove early melanomas, with later stages usually requiring more extensive treatment, such as radiation and chemotherapy. Mohs surgery saves the greatest amount of healthy tissue, appears to reduce the rate of local recurrence and has the highest overall cure rate about 94 percent to 99 percent of any treatment for squamous cell carcinoma. And generally, Mohs surgery is 98 percent or better for basal cell carcinoma, according to the Skin Cancer Foundation.

As the old saying goes, An ounce of prevention is worth a pound of cure, says Dr. Platzer. Say no to tanning beds and always wear sunscreen. Skin Cancer Awareness Month: An Unlikely Patient

When 60-year-old Edna Mayorga first accepted a job with the large cancer care provider, Los Angeles Cancer Institute, a division of 21st Century Oncology of California, she did not know she would eventually become a patient. In November 2012, the 16-year veteran Patient Care Coordinator was diagnosed with basal cell carcinoma skin cancer on her nose.

It started with just a little bump. I thought it was a pimple, but it just kind of grew. Sometimes it would go away and get flat, other times it was like a little bump, and occasionally it would bleed, Mayorga said. That was my sign that something was not right.

A visit to the dermatologist confirmed her suspicions: basal cell carcinoma, the most common cancer in the United States with 2.8 million people diagnosed each year. While this cancer is rarely fatal, if left untreated, basal cell carcinoma can grow into surrounding facial structures. Her dermatologist first referred her to a surgeon to perform Mohs surgery, essentially cutting the cancerous tumor out of her nose.

I evaluated the benefits and drawbacks to the Mohs surgery and the cosmetic effects would be a nightmare for me, says Mayorga. I would end up with a flap then three scars on my nose. I knew there had to be alternative treatments with better cosmetic outcomes. That's when I talked to my own doctors about radiation therapy.

After intense research, she made the decision to undergo radiation therapy with Board Certified Radiation Oncologist, Dr. Daniel Schiffner. Her workdays began to include her regular duties as well as radiation therapy at the Los Angeles Cancer Institute, a division of 21st Century Oncology of California.

I felt confident about starting radiation therapy because I work here and I know it works, she says. But the first time when they close that door, I was a little scared. Dr. Schiffner helped me through and after that, it was no big deal. It was easy and fast with no pain, no poking, or cutting. It was actually a really good experience.

Over time, the basal cell skin cancer was eliminated, leaving Edna cancer free, but more vocal than ever about skin cancer prevention and awareness and a greater appreciation for her patients.

I see things from the patients point of view even more now. Ive always been at the front desk, encouraging them to have faith in themselves and promote positive thinking and positive feedback, she says. Now I can give them more and better feedback because Ive had the experience. That makes me feel even better knowing that I can give my point of view for the patients.

New FDA Proposed Tanning Bed Regulations Push For Improved Warnings During National Skin Cancer Awareness Month

In a recent article published by the New York Times, new proposed regulations for tanning beds in the United States was examined. Expert dermatologist Sabrina Fabi, MD, believes the new regulations could be integral in raising awareness pertaining to the damaging effects of indoor tanning.

Tanning beds may seem like a great alternative to sunbathing, but the same harmful UV rays that are present in sunlight are also found in the lights of tanning beds. This is extremely dangerous for teenagers and adults and increases their likelihood of developing skin cancer, said board certified San Diego dermatologist Sabrina Fabi, MD.

The new FDA regulations that have been proposed would require labels to be affixed to tanning beds warning that use by minors should be restricted. According to the American Academy of Dermatology, 28 million Americans use tanning beds, and of that number, 2 million are under the age of 18. This regulation proposal would be a first step in the direction of restricting the use of tanning beds by minors. The AAD has also stated that the risk of developing melanoma increases by 75 percent for those who have been exposed to ultraviolet radiation from indoor tanning.

Individuals who use tanning beds thoughtlessly expose themselves to UV rays that can lead to precancerous lesions, and eventually, melanoma, one of the most deadly forms of skin cancer, said Dr. Mitchel Goldman, incoming President of the American Society for Dermatologic Surgery.

However, what many may overlook is the importance of a yearly skin exam by a dermatologist and a monthly self skin exam. Those that use tanning beds should certainly reconsider.

All five physicians use Dr. Frederick Mohs technique of mapping by color coding the tissue with dye and removing the skin cancer in layers to ensure that the tissue left behind is cancer-free. Dr. Ciatti shares, We process the tissue in the office at the time of surgery, so there is no suturing done until we know that the tumor is out. Mohs surgery eliminates the possibility of unknowingly leaving some of the cancer behind. We are trying to get the smallest margin possible in order to preserve normal tissue and avoid cosmetic disfigurement, Dr. Ciatti states. As the most exact and precise method of tumor removal, it minimizes the chance of re-growth and lessens the potential for scarring or disfigurement. Dr. Marcus says, Skin cancers are treated in a variety of ways, depending upon factors such as type, location, and age of the person. She says skin cancers can be cut out, burned off, frozen, treated with topical applications, radiation, oral therapies for advanced stages, and Mohs surgery. Dr. Marcus elaborates, A board certified dermatologist will diagnose, biopsy, and recommend the appropriate therapy for YOU. Self-diagnosis should never be an option.

There are many preventable steps to take to live safer under the sun. Dr. Kolansky says, It is strongly recommended to perform self-skin exams on a monthly basis and to visit a Board Certified Dermatologist one to two times a year (depending on skin type and number of moles and sun exposure). It is important to recognize any moles (nevi) that change in size, shape, color, elevate or bleed and should be examined by a dermatologist. The Skin Cancer Foundations recommended prevention tips include: seeking shade between 10 a.m. and 4 pm., covering up with clothing and UV-blocking sunglasses, applying 1 ounce of sunscreen to your body 30 minutes before going outside, keeping newborns out of the sun, and examining your skin each month. Dr. Gold adds, When looking for which sunscreens to buy, look for three things: The number of the SPF The higher, the greater the protection, make sure it blocks out both Ultraviolet UV A rays & UV B rays, as well as look for sunscreens with the words water resistant or sweat proof, which increases a sunscreens performance.

The mercury is beginning to rise across the nation, signaling the long-awaited start of summer. Pools, amusement parks, golf courses and beaches will soon become havens for sun exposure, increasing pubic risk of potential life-long dangers including skin cancer. The Center for Skin Cancer Prevention of the Entertainment Industries Councils Entertainment & Media Communication Institute is sponsoring National Sun Safety Week through this Saturday, June 8th. The week is designed to encourage people to take control of their health by protecting skin and eyes.

The Center for Skin Cancer Prevention, formerly known as the Sun Safety Alliance, provides detailed sun safety resources, activities and information for all ages, demographics, and outdoor enthusiasts. While sun safety should be practiced 12 months a year, we spotlight the summer months when outdoor sports and the desire to tan elevate the dangers of skin cancer for young people. Our goal is to inform every person, through the power of mass media, of the dangers of excessive sun exposure, said Brian Dyak, President and CEO, Entertainment Industries Council, Inc. (EIC).

This week represents eight years since Congress first recognized the importance of sun safety and the need for school-based sun safety education programs. In a resolution by the 109th Congress, all Americans were encouraged to protect themselves and their children from the dangers of excessive sun exposure and to support the goals and ideas of National Sun Safety Week.

Suggestions for being Sun Safe:

Just one blistering sunburn can double a childs risk of developing skin cancer later in life, so make sure to protect your childrens skin and teach them healthy habits.

Skin protection is important even on a cloudy day. Use SPF 30+ on all exposed skin, and remember to reapply every two hours. Make sure that your sunscreen protects against the entire UV spectrum, including both UVA and UVB rays. Dont forget areas such as your ears and the tops of your feet; they can burn, too.

Donning a wide-brimmed hat and sunglasses will protect your familys eyes and face. Seek shade during the hours of 10 AM 4 PM, when the sun is strongest. Remember that water, sand, and concrete reflect the sun, making its rays even stronger.

During extreme heat, check on elderly friends, family, and neighbors, and remind them to remain indoors and stay hydrated. Drinking plenty of water is also especially important when spending time in the sun.

Tanning beds are just as dangerous as the sun. Join the healthy skin movement, and teach your family that fair skin is beautiful and healthy.

The one reason breast cancer gets more than its fair share of attention in the media today is due to the countless efforts of breast cancer activists and celebrities who themselves have announced publicly their own fight and struggle for the disease. One of the more known celebrities, Angelina Jolie, recently announced that she made a choice to have a double mastectomy due to a genetic screening test. This test determined that she is carrying a mutation of the BRCA1 gene, which increases her risks of developing ovarian or breast cancer. Jolies doctors explained that due to this genetic mutation, they estimated that she had a 50% risk of ovarian cancer and 87% risk of breast cancer. Mastectomy is surgery to remove the entire breast. All of the breast tissue is removed, sometimes along with other nearby tissues. In Angelina Jolies case, this procedure will help lower her risk to less than 5%.

Once I knew that this was my reality, I decided to be proactive and to minimize the risk as much I could. I made a decision to have a preventive double mastectomy," Jolie stated to the media. Her aunt and mother had passed away after being diagnosed with breast cancer due to this same genetic mutation. Angelina Jolie was impelled by a desire to encourage other women to get genetically tested and raise awareness of the options available to those at risk.
