The proprietary web-based software, CareClix Home, may be accessed by patients from home via their Patient Portal and this clinical information may be transmitted from their devices such as a blue tooth glucometer for diabetics, creating an alert to a provider remotely. In essence, the Patient Centered Medical Home (PCMH) is taken to the next level, and an integrated delivery system enhances the physician-patient relationship.
